Non receipt of Pan Card

My Pan number is ABWPB0729Q, in the name of Kailash Chandra Bohra. While working as an Executive Engineer Dausa; I applied for PAN number. During the the course of delivery, I was Transferred to Nathdwara. My office at Dausa never responded me about my PAN card or PAN number. It was my persistent querries, I got an extreme faint copy showing my PAN number. Till the date I never got my PAN card. One of my CA had tried and could get a copy of PAN card from IT site, this site now a days is not responding. The copy was extremely faint and my photo is beyond recognisestion. Every year and every where this proof is reguired. I was transferred frequently during my service span. I retired in now it is required every where. I applied twice for duplicate, once from Bhilwara and from Surendranagar (Gujrat). But so far it is in vain. Any body can help me as to from where I can acquire it. Presently I am at KARAD (Maharashtra). My Permanent address is, 1-Ka-1, Chopasani Housing Board Colony Jodhpur.
