Manditate form submit issue

I am sonai muthu from chennai. i bought personal from hdfc in last april. my firt emi passed through in my saving account (icici) to personal loan buyers.but last may 2nd week i received one message in my mobile that message is your sign is miss matched so fill it mantate form and submit the near bank.so i call customer then same procedure they told to me, so i asked near bank collection centre for t. nagar branch at chennai my problem i will not give this form in office hour so i will give the my friend so this is possiple but that branch staff did irresponspile behavier to my friend and same to me
