Non receipt of annuity

Dear sir,

I have annuity noLX0034060 and LIC ANNUITY CARD no. 4011511730011686.It has been isued by LIC OF INDIA, Fort Brach, Mumbai. I am not getting annuity of Rs.410from July. On inquiry they informed that doctor certificate is required to confirm alive of the person that has also been sent 20days back. But no response received till now.
