Duplicate ATM Card

My name is Suraj Raju Shemade (Uttam nagar Pune) I have Saving account in bank of India (main branch camp branch) i had lost my Bank of India's ATM Card. I filled a request form in bank in August. Bank said it may take 45 days. I did not get it. i visited bank on 13 October bank said they do not have my request form, it was not updated in computer. Again, I filled another form I asked for reference no. they provided me. its 31st december now & still I did not get it. When I call bank they say it may take 4 days or may also take 01 year to get my card. I also kept 1000Rs balance in my account so that balance should not b d reason for that.
