FDR not received

My Savings Account no with ICICI Bank is 006401522473 I have Depostied Rs. 40, 000in FD Account by Cheque no 004927 and same has been debited from my Account on 09.04.2012 but till date I have not received the FDR for the Same. Inspite of giving constant reminders to the executive but than also I have not received the same. Kindly do the needfull by sending me the FDR on urgent basis.
