Password missing in the lette

I am holding an account in andhra bank frm a very long time

lastyear i lost my atm card applied for a new one got it also but they didnt give me password to operate it, i was going out of country ask them to give me soon but nothing happened, i went away after three months when i came back it was there in the mail box

and when i opened it it was blank

now what to do

its very urgent

i hv to withdrew cash frm my account dont hv the pass book also

plz advise me what to do now

do i hv to apply again or what
