I had requested for PF withdrawal by sending 3 sets of the forms to ICFAI Hyderabad for that.

The forms wrer forwarded to

Office of the Addl. Central Provident Fund Commissioner (SZ),

3-4-763, Bhavishyanidhi Bhavan,

Barkatpura Chaman, Hyderabad – 500 027 (A.P).

PF account no's were: AP/HY/56966/04913

AP/16939/04389

AP/16939/04388

till now I HAVE NOT RECEIVED MY PF AMOUNT.

I request you to take immediate action on this matter.
