PF fund withdrawal

I have worked with this organization and resigned after 3 months service and the HR told me to contact them after 2 weeks for the withdrawal of fund. When I contacted they were reluctant to give the withdrawal form. Later as per the information from a friend I have downloaded the form and filled up it and went to submit it to the HR.But HR is not ready to talk saying she is busy. They are not at all co-operative. It seems like they are doing some fraud. I am not sure whether they have deposited my money to the EPF account. They are not ready to even tell the PF account number. I would like to know if I can apply for withdrawal online and if they are not giving the PF account number where should lodge a complaint. Who is the concerned officer in Bangalore.
