Dear Sir,

With reference to the mentioned subject, I was submitted my PF claim form to M/s Midland Corporation on 2 year before, but I was not getting any proper feedback from them. Also when I personally meet the midland for PF claim, I observe that our PF claim not yet submitted to the PF office. So we again send our original PF claim Form to Midland through register ad on 3 March 9.

I resign this company long back, but still our PF claim not yet submitted to you, We are request you to kindly withdrawal our PF amount and take necessary action against M/s Midland Corporation.

We are enclosed our copy of PF withdrawal form duly sign by employee of midland corporation, copy of register ed, copy of mail which we were follow-up with them, mail copy of mail send to PF office for follow-up, company leaving letter accepted by me, copy of my salary statement of last month (before leaving company).

We hope that you will take the necessarily action and we will receive our PF amount at the earliest.
