Dear Sir.

SUB: Defaulting Company-non settlement of E P F dues to me

REF: Companys name Amethyst My P F No 50632/48 { Kaushik

Mustafi }

With reference to the above I humbly wish to draw your kind

attention to the following.

I have been employed to the above mentioned company from 01/11 to 19/12 and received a monthly salary. At the time of receiving salary deductions were made by the company towards E.P.F

and the E.P.F number given by the company is as mentioned above. However no E.P.F statements were given to me by the company.

After having served for two years, since I had demanded the E.P.F

statement for the amount deducted from my salary, and for reasons unknown to me, the company were not in position to give me details of E.P. F deductions or E P F statements. Since I continued demanding for the above, I was denied employment by them and told to stop coming to work.

Since that time I have been asked to approach the companys audit office

and I have been there a number of times and unable to get any information from them.

I finally decided to submit my complaint to you directly with all

hopes of just and speedy settlement of the above problems.

Due to certain personnel family problems and financial difficulties I

humbly request you to kindly look into the matter at the earliest.
