New Gas connection is not issued

Dear Sir/Madam,
On 2nd of Aug I had applied for new gas connection at Bharat Gas distributor (Ratan Enterprises, Near Kalewadi Phata, Pune-17), but till today they have not issued the same. I have already submitted the required documents to the distributor at the time of application. I am totally fed up making follow ups with the distributor
And also they are saying, Once approved I need to pay arround Rs 5000/- for single cylinder connection (Mandatory to buy Stove, Lighter...)
Please help me to sort this out.
