Concession in railways for kidney patients

I am a dialysis patient for the past 4 years, i want to know the details given for kidney patients regarding concessions on trains in southern railways, i am finding it hard to find details reg. that even the railway persons are not sure abt the concessions on season tickets as well as long journey trains for kidney patients pls eitther u give the phone number for details whether i can avial any concessions, it is very difficult for patients like us to do dialysis and it is highly unaffordable for persons like me and this extra expenses for travel makes us more burden to our caretakers
