Ref. (Indian Oil) - Consumer No.1837 (transferred from Roop Gas Agency)

This is being to your kind notice that my LPG Indane connection consumer No.1837 in the name of Ajit Kumar Shukla (A.K. Shukla) residence of 199-A&B Gazipur, DDA Flats, Delhi-110096 with Verma Gas Agency, F-6 Pal Market Mandawali, Delhi has been disactivate because of 6 month gap in last booking i.e. 27th of June. As that time I was out of station for official training original papers since 23 years old have not found.

Therefore, you are requested to please instruct to above said agency to activate my LPG Indane connection consumer No.1837 and issue me duplicate paper for my record. At an earliest so that avoid unnecessary problem to collect the LPG Cylinder.
