Change fathers name

I am Jai Sharma. I have passed my class-x in 2011 from Yug Dharm public school Shujalpur Dist- Shajapur (M.P.) My Roll No. is 1200005.My father's name printed in marksheet is Shivkumar sharma it is wrong. My father's correct name is Ashok Kumar Sharma. So i want to change my Father's name in marksheet.please tell me procedure.also please what all documents do i need to submit. I will be higly obliged.plz reply
