Non receipt of PF monies

I had applied for withdrawal of my accumulated PF monies (PF Account No. MH/27657/A/68) from Chetana Institute of Management, where I was employed from August to October (I resigned in Novemebr 2009). This application (along with all requirements) was forwarded to the Institute on 25/06/10. More than six months have lapsed and there is no response from the Institute. I had checked with Bandra PF office and was told that my form had not reached them. This Institute is notorious for such malpractices and harassment, and I fear that I may never get my dues. Please take appropriate action in this regard.
