Missing surname

Dear,

My real name is Avinash Quadras. It is mis spelled in school certificate and also in my passport as Avinash (surname missing). My fathers name is Thomas Quadras but in my passport it Thomas Koradas. I want to make the corection these all mistakes. What should i want to do for that? Please help me ..Please send me a mail regarding my complaint..
