Father's name completely wrong printed

I, T.venkata jagannath passed out MCA int the year 2009 with registered no 06H91F0015 .when i went to college to take my provisional i have seen that my father's name is completely wrong printed on my provisional certificate. This is to inform you that my father;s name is NARASIMAHAMURHTY TATA instead of that it was printed as "APPLANAIDU". So, i request you to please let me know the process of correcting my fathers name in my provisional and also i request you to please inform me whom i should contact in this issue.
