Non issue of receipt

My father r.p.patwari got treatment from dr. s.r.baheti in kolkata for 2 months.i have a mediclaim from national insurance co.i requested the dr several times to issue me proper receipt for the consultation fees paid by me.

but he is not issuing me the receipts. Insurance company is not settling my claim without receipt.
