LIC of India's Health Plus Plan
Regarding Bond

Dear Sir / Madam,

I (Dr. Brij Kishor Gupta) had purchased a LIC`s Health Plus Plan in the last October with risk covering my family i.e. wife, son and daughter and premium was Rs. 20, 000/- yearly from LIC office, Gjaraula, Dist. J.P. Nagar (Uttar Pradesh) under Meerut Division (North-Central Zone). My policy number is 255831506.

I have got premium receipt copy of Rs. 20, 000/- on dated 06/10. But till date I have not received the bond. I told many time to my agent (Smt. Alka Verma, Code-0040325G) but she didn’t done anything. As per my opinion she is unable to help me and solve the problem. Also I have complained to LIC office, Gjaraula regarding bond. I become tired to follow the same.

So you are requested to do needful action against my problem and send bond on my following address as soon as possible.
