Patients papers missing

My father is admitted in KEM Hospital, Ward No. 29, Fever Ward on 06.07. same night his file of case papers was misplaced by a doctor, after 1 hr it has been found. Now again 2=3 days ago his X-ray of chest is missing. How negligence these people are? also since 6th july the doctors are unable to diagnosis him, fever is still coming very often. Kindly look into this matter URGENTLY.I have lodged this complaint, however I know Doctor is always god.
