Doctor is asking for an Extra amount for giving Maternity Reports

This is in reference to the above mentioned subject line, My wife (Mrs. Poonam Giria) was operated in Kumar Maternity Hospital, Pilkhana, Howrah, Kolkata, She was admitted on 15th of July and same day she was blessed with a baby girl. And later after4-5 days she was discharged by giving a simple Reciept of Payment, with some incomplete details.

I am working for Birla Sunlife Insuranc Co. Ltd, Bangalore, and my company provides me the facility to claim such amounts paid towards my family members. When i approached my Department for claim, they raised sum query and documents, accordingly when one of my family member approached to the Doctor Mrs. Srivastava (Phone nos. 033-26769032 & 033-26659253) she denied to give such reports, where she claims that it was not informed to them. And still if you need reports then the cost of Such reports will be Rs 5000/-.

I reqeust your kind help in getting me the reports, where its a doctors duty to give any reports as asked by the patients.
