Hospitalization claim

I have submitted all the related forms through LIC divisional office along with Hospital documents and bills on 25th April but still This TPA is asking to submit the same forms again, these peoples letter date and post date will not match, No body will pick the phone, unnecessarily they will drag the claims and finally they will say that '' we treated the claim as closed.
