Fraud in bill

Some add on plans are getting added to the bill even after placing a disconnect request. The charges get automatically added to the bill. I have already logged complaint but still i have received sms that these charges will get added in every month's bill. whe i have not subscribed on zapakplus.com how will i unsubscribe?
