Wrong name posted in Election ID card

Wrong name Printed in Election ID CARD.

Dear Sir

My name has been wrongly Printed in the Election ID card and so also the same wrong name is reflecting in my Wifes card as my name is stated as husband.

I want to resolve it, because even if I have a card is useless to me because of wrong name posted in that. I too want to change the address of my card because I am staying is in a different place now a days...

Please guide me how to go about, I want to bring this
