ARS bond certificate no 38498738 & Investor Id 215691047 on the name of Yaxi Vipul Makwana Rep by vipul Makwana Please note that I have Recd the cheque after maturity Amount of Rs. 5268 chq no 226572

as pay Yaxi V makwana A/c SB 002401018171 (Please note that this a/c no is my as vipul makwana due to mentioning this a/c no bank was reject the cheque & advise me to open a new a/c of Yaxi Makwana. I have open new a/c on the name of yaxi makwana (Young star - minor) A/c no is 036101000768 ICICI bAnk bopal Branch Ahmedabad)

Now i have requested to issue me new cheque mentioning new a/c no 036101000768

I have already return the cheque
