Dear Sir,

I had a Children's Education Policy of UTI for my minor dependent Nikhil Sharma, which later prematurely terminated by UTI into ARS bonds (Inst. No. 47340991, Folio: 237732871) While redeeming I was sent wrong name on the cheque. Lastly, I was sent cheque no. 119375 of Axis bank dated 19.6.08 for Rs. 16836 which I could not deposit in time.

It is requested to revalidate the cheque in my name DEV DUTT SHARMA, A/C no. 10257713775, SBI, Chandkheda, IRS Complex, ONGC, Sabarmati, Ahmedabad. My dependent Nikhil Sarma is still minor and therefore I can not open an account in his name.
