Not recieved atm card from last 3 months

i had opened my account in M S karnal road kaithal

3 months ago

I have not recieved my atm card from last 3 months . i had filled up the atm form for 2 times. i have also gone to the bank many times, but they are not responding and even they didn't talk properly said that again fill up the form and i also filled up the form 25 days ago but till now i have not recieved my atm card

PLEASE take appropriate action so that in can get my atm card plzzzzz si
