I not getting my Monthly Statement

Dear Sir,

I have Axis bank credit Cared post 2 month. CARD No 4718630100784056 But i am not getting monthly bill statement last month and this month also. I was tell so many time in your Telephone Excutive, but still i am not getting my bill, my payment last date 15th of month. I will me pay the cash. So plz help me and send the bill in my mail ID as well as my communication address also.
