Wrongful Debit From Salary a/c no 636010100022765

Dear Sir,

Unfortunately I have lost my Debit card on dated 16 May. The founder has used my debit card no 4215726360001203 and purchased mobile phon/accessories as per Axis bank statement PUR/MOBILE POINT/ 913715565317/170509/21 and PUR/MOBILE POINT/913715475114/170509/21 from Mobile point shop on dated 17.05.2009.

As per ethics/rules of the banks the signatures are always being verified before any transaction is made.

Once the founder has purchased the above said items by swapping the card and signing the purchased slip.

Ultimately the slip might have come to Axix Bank Indirapuram Ghaziabad (My Salary A/C Bank). It is sure that the signature might have not been verified by the bank staff and blindly transferred the amt. (Rs. 12607 1887=14494/-) which is a big amt for me. May I request you sir, to help me to get my said amt refunded in my above said a/c.
