Not issuing ATM card

Hello Sir,

I have opened account in Bank of Baroda's Bhatar Branch in Surat on the name of MRS. CHANDRIKABEN YOGESH BARDOLIWALA and the a/c no. is 25260100012191 dated 09-07. I have filled the form of ATM card on same date. They told me that u will got your ATM card within 20 days. After 20 days when i go to the branch they told me that your reqest is not sended, you fill another form. So i have filled another form on the dated 29-07. Now after 23 days, i go to the branch they told me that your card is not issuied yet. I reqest you to please do something in this matter.
