Delay in sending Cheque book

This has reference to my saving account with bank of india i had given request for issue of cheque book at bank of india kandivali branch on somewhere around 04.07.2009 as informed to me the same has been couried on 14.07.2009 which has not yet been recd.. On contacting the concern branch relevant authority they started passing the balls on courier company and head office. I email to head office at bandra but no action has been taken.
Who should be resposible for all this? Branch Manager who was not available or Head office?
