Membership Charge - Additional benefits

I have been charged sum of Rs. 6, 741.60 against some membership charges. I had called up the bank and instructed them to cancel this policy but they cannot cancel the membership policy. I have even not received the statement or any kind of vouchers and they keep on sending the message regarding the POD.

Would request you to please look into this matte
