Non issue of regulato

I got one connection transferred from M/S Milind gas service (code-581852) N delhi-18 on 22-08-06 to M/S Prakash bharat gas (code no-140740). Once the regulator knob got stuck and on giving extra pressure it had broken but the regulator was not replaced by Prakash bharat gas since the no. of regulator issued by previous distributor was not written on the SV issued previously. Checklist for the consumer before receiving SV is nowwhere seen which would have been confirmed. My consumer no. is 6199.
