Y mistake of retailer some is using my sim card

My name is MD.IMTIYAZ ALAM S/o MD .NIZAMUDDIN ADD:- PANDRA KANALI PURVEE, DHANBAD (JHARKAHAND) i bought a sim card about 2 years ago on id proof of mine after a year i found that the sim card i am using is not on my name no, 9006881908 and somehow i managed to have the registered name i discuss with the retailer from where i took this card i told him about this problem but he kept faking that he would solve my problem. even i don't know the no. registered on my name i want block that no please that no. or tell me how it could be done...
