Fordge signature and medicals

Dear manager,

I had applied for a policy in Birla sun life insurance company.my policy num is (002849822)... the documents which i recived from the company having my fordge signature and fordge medicals reports. I am residing in ludhiana nd my medicals reports has prepared from jamu.and the deatail of my nominee is also wrong.i dont want to continue wid this policy.i had given my policy cancellation leatter in loacal birla sun office on 25 june but till date they are not telling me the status of my policy.kindly register my complaint and refund my poilcy..
