Delay in service

Dear sir/madam,

I have taken a telephone connection with broadband and my telephone number is 23437023 and connection in the name of Shanti Devi. The time when i took the connection there was not type 2 modem available with telephone exchange it was told by them that once type 2 modem come it will be provided to me. I have inquired about that type 2 modem is available but the same has not been provided to me. And i have also inquired Ms. Uma is the concern person, but she will not be available neither at office nor at mobile/phone number given to me. Please take a proper action against this.
