Poor Broadband connectivity

I had got my broadband connection on April. MY telephone no. is 25654658 is working properly. But broadband is not working properly. My modem is D-Link ADSL. It showing 678, 691, 619 & more. The main problem is not continuous link in modem. But I confirm that there is no problem in my modem. It working properly in other line.
