SIM CARD/MOB LOST - REG

My above mentioned mobile (Prepaid) sim issued by Vyara exchange, Tapi district, Gujarat, has been lost/misplaced during my visit to Mumbai during May. I am unable to locate the same. I request you to please block my mobile/sim and reissue the same to me at the earliest.
