Broad band bill issued to my number, but my broadband was never installed

I got a new connection for BSNL in Indrapuram (0120-2605810). I received a bill of 38883 750, 750 was for broadband. Broadband never got installed. I have initially filled the broadband form but nothing happened.

There after I went to Indirapuram exchange so they asked me to fill the form again, but then I shifted to TATA for broad band as BSNL service was pathetic. In March I got a healty bill with extra debits of 2550 and 975 as deposits. I could not understand the debit details.
