Phone is dead

Dear Sir/Madam,

I have basic telephone No. 2604611 by name of Smt. Kanti Devi at 127/575 w-1 saket nagar, kanpur

I am so puzzled due to registered complains many times to saket nagar telephone exchange but there is no response and my phone is dead from last three months and your deptt. has raised a bill while my phone is not working from last three months.

So, I request you please resolve my problem otherwise this conversation consider as a application for disconnection of my phone.
