CAF given but not start my mobile no

Sir, my prepaid mobile no. is 9454896839 (UP (E) Bahraich SSA). my mobile was cut due to non submitting of CAF but i have given my CAF on 03.08.09 but it not still working. Mr. I.M.Rao SDE (Mobile) O/o TDM Bahraich BSNL is incharge of this Section.
