Non submmission of cellone prepaid form

I have already submitted all required documents like zerox copy of photo ID card, age proof, ration card, address proof etc. But distributer of my area told me that those have not been submmitted to your office and so that my mobile no.9428884852 will be canceled by your compony. I want to know that which document has not been submmitted and what should i do to continue my mobile service? should i send all documets again or otherwise?
