Delay in the Passport issue

My name is Surinder Kaur. I am a Govt. Employee in Teaching Department. I have applied for the passport on 31-07 and not received yet. The Status is still Police Report not received... but the clearance was send the teaching department.

Date of birth is 15-09-1956

while correct DOB id 5-09-1956.
