Dear Sir,

I wish to draw the following facts for your kind information and necessary action:-

1. I was employed with M/s. CAMLIN LTD, Mumbai from 1985 to 2010. I resigned from the company services w.e.f. 14/10.

2. It may please be noted that I have submitted all the related papers regarding Provident Fund & scheme certificate under Employees' Pension Scheme (1995) etc to your office through my employer.

3. Please note that i have received my Provident Fund on 27th May through ECS in my bank A/c. But till date i have not received any statement regarding my PF accumulation.

4. Also note that the necessary documents for scheme certificate under Employees' Pension Scheme (1995) submitted to your office on 25th January through my employer, but till date i have not received any certificate nor any communication from your office.

5. I have contacted your office constantly by telephone but not received any authentic reply.

It would be highly appreciated if you could help me in this matter.
