Mis-use of Card

I am holding Card No.4339481144206949 (which I have blocked today in the early moring say about 3.50 a.m. of 25.09.2009) I was informed by your Customer care department that have I incurred a transaction of about $ 119 or 109 today, I replied in the negative. I have to inform you I did not use the Card for the past one month or so nor have I made any transaction as told by your customer care executive in the early hours today. I therefore request you earnestly to look into this matter as nothing of such nature has ever happened with me. I am also forwarding Dispute Declaration Form duly filled in for necessary action at your end. Kindly advise me whether I should send the form forthwith or should I wait till the Statement of Card is generated and conveyed to me.
