PF Cheques not received from Chennai PF office

Request you to resolve the matter regarding my the PF cheques ASAP.

I have been following up this matter since Jan 08 but so far didn’t receive any positive feedback from PF Chennai office.

As directed by your office, my ex-employer Polaris Software Lab & myself had already sent the required letters dated 10th March 08.

I am again attaching the copies of the letter send for your reference.

Hoping this time the matter gets resolved & I get some positive feedback.

The details of the problem are mentioned below:

PF account no: TN/31140/5525

Cheque Date Amount

698742 23/11 20150.00

183163 26/11 60079.00

The above-mentioned cheques were drawn on the HDFC SB ac no 5421050027969 (Ac name –VAISHALI PATIL), which belongs to my maiden name.

The cheques were returned to PF office as it was drawn on my current name VAISHALI MARTIRES. Please reissue the cheques as requested several times before.(The account name has been changed to my martial name now)
