Respected Sir/Madam,

I am Eshwar Raj, with reference to the above subject I have appointed a nurse for my father from M/S. CHETHANA HOME NURSING. Popular Building, 2nd Floor, Main Road, Kundapura - 576 201. Ph. # 08254-232279 on 02.08.2009. As per there charges we have paid Rs.5000/- against Inv # 308. The Invoice was in the Patient name i.e., my father name M.C.Vasanth Kumar. But that nurse has ran away on 13.08.2009 from our house with out intimating us simultaneously she has stolen my Father’s Mobile worth Rs.4000/-. But she have not carried her bag..

From Three Months we are requesting the Proprietor of that company Mr. S.B.Shetty (Bhaskar Shetty) M # 9845252093 to arrange another nurse are to pay the amount. But the person is not responding properly from 15 days he is not answering the mobile also.

So i request you to take immediate action in this regard.
