No closure of citibank suvidha account

I have requested many times to close the account and also fulfill the requirement to close the account but they did not did. i have not ant details bcz i have sent my debit card as well as check book to citibank. they have applied monthly charges. bcz i withdraw all my money from my account.pls tell me what should i do?
