Simcard problem

I had purchased a sim card on 14.03.08 for which i submitted the required documents on time. but after a week my services were cancelled reason being dat i have not submitted local address proof. The dealer had asked me to fill form number 60 as per the above requirement . its been almost 3 weeks my sim card is still out of service. same thing has happened with my other new sim card also which i have purchased a week back.

iv been trying to contact the customer care executive but every time my call gets disconnected.
