Poor service

Dear Sir,

My landline No of Baijnath exchange is 263834 got faulty since 14 Apr. I have booked the complaint in several time. BSNL Baijnath field staff visited the site two to three time, they informed that this major problem in UG. They told that fault of the your phone could not rectified and WLL can be provided to you. On 16 June I personally met with SDO Baijnath he told me that your problem will be solved within two days, but as on 20 June no action has been taken so far. My wife went to BSNL Baijnath Office and asked for disconnection of telephone connection and demanded for form. The staff told her that we have no form and you write the application on plain paper. Please guide me for further action please.
